When completing the referral form, please provide the following:

 

Request for Testing Services: Indicate if testing services are required and include any relevant information supporting the need for these services.

 

Supporting Documentation: Attach any records that substantiate the client’s current diagnosis and treatment history, particularly those most relevant to the testing request (e.g., the most recent office visit notes detailing the level of current impairment or concerns).

 

While we do not conduct in-house testing, we have a network of qualified professionals who can carry out the necessary assessments. We will review the referral information promptly and determine how we can best accommodate your request by arranging the appropriate testing.
